AFFF Spill at Hawaii Navy Base Alerts Public to Dangers of Fire Fighting Foam Chemical Exposure to Humans

According to the Navy, the reason for the terrible and toxic AFFF spill was “human error.” The Navy asserts that it hired a maintenance contractor that improperly installed an air vacuum valve during a modification to the facility’s AFFF system. That modification was made in April, 2022. The modification that was made to Red Hill’s AFFF system, in combination with the same maintenance contractor’s failure to disable AFFF concentrate pumps from “automatically starting prior to running tests on the fire suppression system” caused the AFFF spill in late November last year. According to reports, the maintenance contractor did not notice the AFFF leakage until about 20 minutes after it occurred.
In response to the dangerous AFFF chemical spill United States Senator Mazie Hirono issued the following statement: The Joint Task Force Red Hill investigation found that the hazardous AFFF spill at Red Hill was caused by human error and was preventable.
